Applications are currently being accepted for the Wyoming Youth ChalleNGe Program class beginning July 2007.  Don't delay.  Please fill out this form in order to pre-apply.  Once the pre-application form is received, a staff member will contact you and send an application.

Documents Required at In-processing
You will need to make certain that you have copies of the following documents at in-processing.

• Birth Certificate
• Photo I.D. (Driver's License or State I.D. Card)
• Social Security Card
• Medical/Dental Insurance Cards
• Immunization Records (must be current on all vaccinations)
• School Transcripts
• IEP - Individualized Education Plan
